IUA / what you pay before sharing
Published IUA options are $1,250, $2,500, $5,000. That amount is your per-need responsibility before the community shares. Co-share after the IUA is listed as 10-20% in the plan file. Sharing cap: Unlimited per need; no annual or lifetime cap. Pre-existing conditions wait 12 months (then phased in over 4 years) except the named day-one carve-outs for managed high blood pressure, high cholesterol, and type 1 and type 2 diabetes when there was no hospitalization in the prior 12 months.
What’s not in the monthly share
Routine dental and vision are not shared. Maternity has its own timing and IUA rules — read does Zion HealthShare cover maternity instead of inventing a maternity dollar here. Pre-existing needs that are not in the day-one carve-out list wait 12 months, then phase in over four years. Zion is not available in WA. Processing is listed as 30-45 days. Sharing is never guaranteed.
